Dental Insurance

Provides coverage for a variety of dental procedures, from routine cleanings to major services. Additional savings are available by visiting a network dentist at more than 335,000 access points nationwide. Choose from two PPO plans that work in addition to existing dental plans and Medicare. Full Vision Plan is also available. Guaranteed Issue.

Accident Insurance

Helps offset unexpected medical expenses, such as emergency room fees, deductibles and co-payments, that can result from a fracture, dislocation or other covered on-job or off-job accidental injury. Non-fatal gunshot wound benefit of $10,000 is included. Guaranteed Issue.

Critical Illness Insurance

Supplements your major medical coverage by providing a lump-sum benefit you can use to help pay for the direct and indirect costs related to a covered critical illness, such as heart attack, end-stage renal failure, coronary artery bypass surgery, stroke or major organ transplant. Lump Sums range from $5K-$50K. $10,000 Guarantee Issue amount regardless of health.

Whole Life Insurance

Provides Guaranteed features such as cash accumulation, premium rates, along with the death benefit. Guarantee issued amounts available based on age. Keeps the family lifestyle intact in the event of your death.

How do I enroll?

Simply click on the scheduling links on this page to schedule a date and time at your convenience. A benefits counselor will call you at your scheduled time to review your benefits options and answer all of your questions.

Can I cover my family?

Family Members can be enrolled in all plans.

Are there medical exams?

There are no Medical Exams needed for Dental or Accident Plans. Should you purchase Critical Illness and Life Insurance and exceed the guaranteed issued amounts, medical questions or a medical exam may be necessary.

How do I pay for this?

At the time you purchase the policies, the Benefit Counselor will obtain banking information. This will be deducted by Piedmont Pay Inc. at no cost to the member. Piedmont Pay will then forward your premiums to Colonial Life.

How do I file a claim?

Go to Coloniallife.com and open an account. This will enable you to view your coverages, fill out claim forms, and allow you to complete the process. You can also call 800-325-4368 and speak to a customer service representative to initiate a claim.
