How do I enroll?

Simply click on the scheduling links on this page to schedule a date and time at your convenience.  A benefits counselor will call you at your scheduled time to review your benefits options and answer all of your questions.

Can I cover my family?

Family Members can be enrolled in all plans.

Are there medical exams?

There are no Medical Exams needed for Dental or Accident Plans. Should you purchase Critical Illness and Life Insurance and exceed the guaranteed issued amounts, medical questions or a medical exam may be necessary.

How do I pay for this?

At the time you purchase the policies, the Benefit Counselor will obtain banking information. This will be deducted by Piedmont Pay Inc. at no cost to the member. Piedmont Pay will then forward your premiums to Colonial Life.

How do I file a claim?

Go to Coloniallife.com and open an account. This will enable you to view your coverages, fill out claim forms, and allow you to complete the process. You can also call 800-325-4368 and speak to a customer service representative to initiate a claim.
